The Additional Needs and Disabilities (AND) Partnership has designed a survey for all parents and carers of children and young people with AND in Surrey.

We want to understand your experience of accessing help and support related to Special Educational Needs and Disabilities (SEND) in Surrey. We will use your feedback to improve local services that support Surrey's children with additional needs with their education, health (including diagnosis) and care.

The survey closes at 23:59 on Sunday 24 November 2024, and only one entry per person will be valid.

The survey will take around 15 minutes to complete and your answers will be anonymous.

It is optional to complete the survey or not. This will not affect your access to any Council support or services.

As a thank you for completing the survey, you could be in with the chance of winning one of four high street shopping (One4all) vouchers each worth £50. At the end of the survey, you will have the option to enter the prize draw.

  • Closing Date: Sunday 24 November 2024 (midnight)
  • Entrants: UK residents aged 18 years or over, except employees of the Promoter. or anyone directly associated with the administration of the Prize draw
  • Promoter: Surrey Additional Needs and Disabilities Partnership, Quadrant Court, 35 Guildford Road, Woking, Surrey GU22 7QQ
  • How to enter: Submit contact name and email and/or telephone at the end of the survey to be entered into the Prize draw – details at the end of the
    survey.
  • Prize(s):4 x £50.00 'One4All' vouchers

1. Only one entry per person is allowed.

2. The Prize draw is free to enter and no purchase is necessary.

3. All entries must be submitted via the following survey on Surrey County Council's website (https://online1.snapsurveys.com/scc-parentsvoicematters2024) or via one of the survey's accessibility options by following the relevant instructions.

4. Only entries submitted before the Closing Date will be accepted.

5. The Promoter accepts no responsibility for entries not successfully completed due to a technical fault of any kind.

6. Winner(s) will be chosen by random Prize draw performed by Surrey County Council on Wednesday 4th December 2024.

7. Winner(s) will be notified by email or telephone (using details provided at entry) by Friday 13th December 2024. If a winner does not respond to the Promoter within 14 days of being notified by the Promoter or the winner is unable to or refuses to accept the Prize, then the winner's Prize will be forfeited, and the Promoter will be entitled to select another winner in accordance with the process described above.

8. Where the winner has provided their address, the Prize will be sent to the winner by post within 28 days of being notified on their win.

9. Prizes are non-exchangeable, and no cash alternative is offered.

10. The Promoter reserves the right to substitute the Prize with an alternative prize of equal or higher value if circumstances beyond the Promoter's control makes it necessary to do so.

11. The decision of the Promoter regarding any aspect of the Prize draw is final and binding and no correspondence will be entered into about it.

12. The Promoter will make available information that indicates that a valid award took place by either sending the surname and county of major Prize winners to anyone writes to the Promoter at the address above enclosing a self-addressed envelope and
stating the name of the Prize competition within one month after the Closing Date. If you object to your surname and county being made available, please contact the Promoter. In such circumstances, the Promoter must still provide the information and winner details to the Advertising Standards Authority on request.

13. Entrants are deemed to have accepted and agreed to be bound by these terms and conditions upon entry. The Promoter reserves the right to refuse entry or refuse to award the Prize to anyone in breach of these terms and conditions.

14. Insofar as is permitted by law, the Promoter, its agents, or distributors will not in any circumstances be responsible or liable to compensate the winner(s) or accept any liability for any loss, damage, personal injury, or death occurring as a result of taking up the Prize except where it is caused by the negligence of the Promoter. Your statutory rights are not affected.

15. Entrants' data will be collected, stored, and processed for the purposes of administering and assessing Prize draws. The Promoter shall take all reasonable care to ensure that it complies with its obligations under the Data Protection Act 2018.

16. In the event of fraud, abuse, or an error affecting the proper operation of the Prize draw, including the allocation of more Prizes than are available, the Promoter reserves the right to end or suspend the prize draw; declare void the notification of winner; and re-allocate available Prizes by conducting a new Prize draw.

17. If any provision of these terms and conditions is held invalid by any law, rule, order, or regulation of any government, or by the final determination of any court of a competent jurisdiction, such invalidity shall not affect the enforceability of any other provisions not held to be invalid.

18. The Promoter reserves the right to suspend, cancel, postpone, or amend the Prize draw and/or review and revise these terms and conditions.

19. The promotion is governed by English Law and is subject to the exclusive jurisdiction of the English courts.
