Who to contact

You can find the contact details of your district or borough council, locality team or Home Improvement Agency below.

Find out if your child/young person is eligible for an Occupational Therapy (OT) assessment.

If you are a child or young person being abused or neglected, or an adult who is concerned about a child or young person's safety and wellbeing, you can contact the Children's Services 'Single Point of Access' between 9am to 5pm, Monday to Friday.

There may be a wait before your child/ young person's Surrey County Council OT referral can be allocated to an OT from the Surrey County Council Children with Disabilities Occupational Therapy (CWDOT) Team for a full OT assessment. The CWDOT assessment will be allocated to a CWD OT for assessment as and when appropriate. It is recommended that you maintain contact with the duty CWD OT team (cwdot.duty@surreycc.gov.uk) during this time to alert them to any changes or developments in your child's/ young person's needs at home that may require more immediate OT intervention. If you feel you're not able to wait for a CWD OT assessment, then you can self-fund a private OT assessment.

The children's health OT service assesses children/ young people's needs in school and are community based. They can be referred to using the OneStop Children's Occupational Therapy website.
