Children with disabilities outreach service
The Children with Disabilities (CWD) Outreach Service provides short breaks for disabled children, young people and their families in their home and in the community.
They support children and young people with complex needs, including autistic spectrum conditions, challenging behaviour, profound and multiple learning disabilities and associated complex health needs.
The team is made up of a Registered Manager, Coordinators, Senior Support Workers, and Bank Support Workers. Workers are trained in Makaton, Sensory Integration, Manual Handling and as Autism Champions.

Types of support
Support can be in your own home or in the community. We take a person-centred approach to our work, involving support with a range of skills and activities for example:
- Before and after school routines
- Personal Care
- Promoting independent living skills e.g., cooking
- Swimming
As well as social and leisure activities, such as:
- Bowling
- Horse-riding
- Trampolining
- Bike Riding
- Scouts·
Referral process
Children supported by CWD Outreach must meet criteria as set out in the Surrey County Council Children with Disabilities Services Eligibility Criteria for assessment by the CWD Service. The eligibility criteria are available in the Surrey Children's Services Procedures Manual, within the 'Children in Particular Circumstances - Children with Disabilities' section of the document library.
The children must also have Children with Disabilities social worker.
You and your Social Worker identify that you want to access our service. Your social worker will seek funding. If funding is agreed, they will make a referral to the CWD Outreach Service.
The CWD Outreach Service will contact you to arrange a convenient date and time to meet and introduce the service to you.
They will establish how and when you would like your support and complete a Person Centred Plan and a Risk Assessment.
Once an appropriately skilled worker has been identified, the CWD Outreach Service will arrange an introductory visit so you and your child can get to know the worker before support begins.
Support workers are linked to children and young people based on their skills, training and availability.
Support is always arranged through communication with the Coordinators and Manager.
