Learners' Request for Statutory Education, Health and Care Needs Assessment (0 to 25 years)

Please note, the forms below are for use by professionals working with young people in Surrey.

Parents, carers and young people should not use these forms to request EHC needs assessment.

If you are a young person or are the parent/ carer of a child or young person and would like to request a statutory EHC needs assessment, you can find out how to do so using the form and guidance on the Education, Health and Care (EHC) plans page.


Requesting statutory EHC needs assessment as a professional/ practitioner

Sometimes a child or young person's needs are complex and an assessment of their Education, Health and Care needs may be appropriate.

If you are a professional supporting a child or young person, where you feel a statutory assessment may be required, please complete:

Once completed, please submit these to our team in the L-SPA, using the Learners' request for assessment form.

Before submitting your application, you will be prompted to supply additional evidence to support your request.

Please note, that the online portal cannot accept zip (.zip) files. If you have additional documents that you cannot upload, email them to learnerssupport.triage@surreycc.gov.uk.

What happens next

Our team of multi-agency professionals will review your request. They will make a recommendation whether a statutory assessment is required, based on the child or young person's needs. We will always let you know the outcome within six weeks of the request.

There may be occasions where we do not agree that a statutory assessment is required. However, we will always provide you specific and individual reasons for this, as well as signpost you to services that may be more appropriate.
