EHC plan templates
On 6 June 2022 Surrey introduced new templates for Education Health and Care (EHC) plans. This was done to make the plans easier to understand more visual.
The biggest change to the EHC plan paperwork is that in the year a child turns 14, their plan will move to a 'Preparing for Adulthood' or PFA format. This will mean that we can focus the outcomes on support that will enable them to become as independent as possible in their adult lives.

Applying for an EHC needs assessment?
If you are practitioner working with a child or young person in Surrey who you believe requires an EHC plan, with parental consent you can request an EHC needs assessment through the Learner's Single Point of Contact (L-SPA).
